What is actually going on
An erection is a vascular event. Arousal triggers blood flow into the spongy tissue of the penis; anything that disrupts blood vessels, nerves or hormones can block that process. The most common drivers are cardiovascular: narrowed arteries, high blood pressure, raised cholesterol and type 2 diabetes each damage the small vessels that feed an erection. Smoking compounds the effect significantly. Psychological causes, performance anxiety, stress, depression, are just as real, and often layer on top of a physical problem, making it worse.
How ED is treated
PDE5 inhibitors are the standard first treatment. Sildenafil (the original, taken an hour before sex) and tadalafil (longer-acting, also available as a daily low dose) are the most widely used. Vardenafil and avanafil are alternatives for men who want a faster onset or different side-effect profile. All these medicines work by enhancing blood flow in response to arousal, they do not create an erection without it.
Lifestyle changes that move the needle
Weight loss, regular aerobic exercise, quitting smoking and cutting alcohol can each improve erectile function, sometimes substantially. For men in whom ED is an early signal of cardiovascular risk, addressing those risk factors is not optional, it matters for overall health, not just sexual function.
New or worsening ED alongside chest pain, breathlessness, or known heart disease warrants a prompt medical check.
